On a side view, two curvatures can be seen. The upper back extending from shoulders to the chest-rib cage has a curvature called &#8216;kyphosis&#8217;, on contrary the lumbar area has a curvature called \u2018lumbar lordosis\u2019. These curvatures enable head and upper body stand balanced over the pelvis.<\/p>\n<p>The normal thoracic spine has a slight kyphosis between vertebrae 1 and 12 with an angle of 20 to 45. It is called \u2018hyper-kyphosis\u2019 when the curvature of upper spine is greater than 45 degrees. Scheurmann&#8217;s kyphosis is a typical hyper-kyphosis and formed as a result of wedging of vertebrae in adolescence. The cause remains unknown but is believed to be multi-factorial and it is more common in men.<\/p>\n<\/div><div class=\"btClear btSeparator topSmallSpaced noBorder\" ><hr><\/div><header class=\"header btClear medium btDash bottomDash  btAccentDash\" ><div class=\"dash\"><h3><span class=\"headline\">Medf\u00f6dd kyfos<\/span><\/h3><\/div><\/header><div class=\"btClear btSeparator topExtraSmallSpaced noBorder\" ><hr><\/div><div class=\"btText\" ><p>Sidovyn av normal ryggrad p\u00e5 den direkta radiografin liknar en rektangul\u00e4r form. Den fr\u00e4mre delen av ryggkotorna i br\u00f6stryggen \u00e4r mindre \u00e4n dess bakre del (f\u00f6r att bilda en kyfos). L\u00e4ndkotan har en omv\u00e4nd struktur f\u00f6r att bilda en lordos. Vid medf\u00f6dd kyfos \u00e4r kr\u00f6kningen st\u00f6rre \u00e4n den f\u00f6rv\u00e4ntade lokala kyfosen.<\/p>\n<p>Patienterna med medf\u00f6dd kyfos och skolios har sannolikt en defekt i andra organsystem. Efter verifiering av diagnosen p\u00e5 r\u00f6ntgen kan ytterligare tester kr\u00e4vas f\u00f6r att unders\u00f6ka anomalier i ryggm\u00e4rg, hj\u00e4rt- och gastrointestinala system. MRT (magnetisk resonans) \u00e4r anv\u00e4ndbar f\u00f6r att utv\u00e4rdera onormal tillv\u00e4xtpotential och utveckling av ryggradsdeformitet.<\/p>\n<p>Styrkan hos barnets ben och s\u00e4ttet han\/hon g\u00e5r b\u00f6r utv\u00e4rderas kliniskt. Progressiv kyfos orsakar ett starkt tryck p\u00e5 ryggm\u00e4rgen och kan resultera i kompression av nervr\u00f6tterna fr\u00e5n ryggm\u00e4rgen och myelopati (deformitet av strukturen i ryggm\u00e4rgen). Barn med nervkompression kan bli f\u00f6rsenade att b\u00f6rja g\u00e5.<\/p>\n<p>Patienter med medf\u00f6dd kyfos har segmenteringsdefekt av kotor eller formationsdefekt. Missbildningen fortskrider ofta n\u00e4r barnet v\u00e4xer. Progression av missbildning \u00e4r h\u00f6gre under det f\u00f6rsta levnads\u00e5ret, s\u00e4rskilt beroende p\u00e5 den snabba utvecklingen av skelettutvecklingen. Segmenteringsdefekt har l\u00e4gre framsteg och kanske inte kr\u00e4ver ett kirurgiskt ingrepp f\u00f6rr\u00e4n i ton\u00e5ren.<\/p>\n<h4>Behandling<\/h4>\n<p>Observation:\u00a0Observation \u00e4r f\u00f6rstahandsbehandlingsmetoden f\u00f6r barn med ryggradsdeformitet. Skillnaden mellan direkta r\u00f6ntgenbilder m\u00e4ts och signifikanta f\u00f6r\u00e4ndringar registreras n\u00e4r patienten bes\u00f6ker unders\u00f6kningen. Allvarliga och progressiva medf\u00f6dda kyfosdeformiteter som \u00e4r st\u00f6rre \u00e4n 45 grader eller kyfosfall \u00e5tf\u00f6ljda av neurologisk svaghet kr\u00e4ver ofta kirurgisk behandling. Tidigt kirurgiskt tillv\u00e4gag\u00e5ngss\u00e4tt ger vanligtvis det b\u00e4sta resultatet och f\u00f6rhindrar progression av kr\u00f6kning. Proceduren f\u00f6r att kirurgiskt ingrepp ska utf\u00f6ras \u00e4ndras baserat p\u00e5 deformitetens karakt\u00e4r<\/p>\n<p>Korsetering: Korsettering rekommenderas inte f\u00f6r behandling av medf\u00f6dd kyfos. Kliniska studier misslyckas med att bed\u00f6ma resultaten fr\u00e5n patienter som har f\u00e5tt denna typ av behandling.<\/p>\n<p><strong>In situ fusion:<\/strong>\u00a0Det kirurgiska alternativet f\u00f6r progressiv kyfotisk deformitet \u00e4r full fusion eller artrodes av deformerad kota (f\u00f6rening av kotor). Fusion b\u00f6r ocks\u00e5 inkludera de tv\u00e5 normala kotorna, den under och den ovanf\u00f6r. Bentransplantatet fr\u00e5n patienten eller v\u00e4vnadsbanken placeras baktill (p\u00e5 baksidan). Vid allvarligt vinklad kr\u00f6kning kan ytterligare bentransplantat placeras anteriort (p\u00e5 framsidan). 4-6 veckor beh\u00f6vs tills full fusionsv\u00e4vnad bildas genom bentransplantat. Patienten b\u00f6r st\u00f6djas med gips eller korsett under denna period tills den fullst\u00e4ndiga sammansm\u00e4ltningen \u00e4r klar.<\/p>\n<p>Instrumenterad fusion och osteotomi:\u00a0Progressiva kyfotiska deformiteter kan kr\u00e4va instrument (stavar, krokar och skruvar) hos \u00e4ldre barn. Om ryggm\u00e4rgen inte kan tolerera f\u00f6rl\u00e4ngningstekniker f\u00f6r ryggraden, kan avl\u00e4gsnande av ryggkotan som orsakar deformiteten \u00f6verv\u00e4gas. Den postoperativa anv\u00e4ndningen av korsett och gips \u00e4r densamma tills fusion uppt\u00e4cks p\u00e5 de periodiskt tagna direktr\u00f6ntgenbilderna.<\/p>\n<p>Din kirurg kan ocks\u00e5 rekommendera dig en osteotomi f\u00f6r att justera ryggraden. &#8220;Pedincule subtraktion osteotomi&#8221; ger korrigering genom att ta bort benet p\u00e5 baksidan av kotan. Kotor resektion osteotomi kan appliceras f\u00f6r att ta bort den fr\u00e4mre och bakre delen av kotan fr\u00e5n baksidan. Din kirurg kan ocks\u00e5 rekommendera olika operationer inklusive fr\u00e5n framsidan och baksidan av ryggraden. V\u00e5rt center har en v\u00e4rldsomsp\u00e4nnande erfarenhet av ingrepp f\u00f6r borttagning av kotan fr\u00e5n ryggen.<\/p>\n<\/div><div class=\"btClear btSeparator topSmallSpaced noBorder\" ><hr><\/div><header class=\"header btClear medium btDash bottomDash  btAccentDash\" ><div class=\"dash\"><h3><span class=\"headline\">Utvecklingskyfos\r<br><\/span><\/h3><\/div><\/header><div class=\"btClear btSeparator topExtraSmallSpaced noBorder\" ><hr><\/div><div class=\"btText\" ><p>Hyper-kyphos (fram\u00e5tb\u00f6jning av br\u00f6stkotorna bortom normala gr\u00e4nser) klassificeras som posturell eller strukturell. Postural kyfos kommer att korrigeras n\u00e4r patienten st\u00e5r uppr\u00e4tt. Patienter med postural kyfos har inga avvikelser i formen av kotorna.<br \/>\n&#8220;Scheuremanns kyfos definieras som stel (strukturell) kyfos. Det \u00e4r den vanligaste i \u00e5ldrarna mellan 12 och 15. Den onormala kyfosen ses b\u00e4st fr\u00e5n sidan i fram\u00e5tb\u00f6jd position.<\/p>\n<p>Patienter med &#8220;Scheuremanns sjukdom&#8221; uppvisar ofta d\u00e5lig h\u00e5llning och besv\u00e4r av ryggsm\u00e4rtor. Ryggsm\u00e4rtor \u00e4r det vanligaste besv\u00e4ret under den tidiga ton\u00e5ren och minskar n\u00e4r de n\u00e4rmar sig vuxen \u00e5lder. Sm\u00e4rtan f\u00f6rv\u00e4rras med dagliga aktiviteter. Kyfosen \u00e4r vanligtvis symtomatisk och spetsen \u00e4r i mitten till nedre delen av ryggen ist\u00e4llet f\u00f6r \u00f6vre delen av ryggen.<\/p>\n<h4>Behandling<\/h4>\n<p><strong>Observation:<\/strong>\u00a0Observation rekommenderas ofta i f\u00f6ljande fall:<\/p>\n<p><strong>Postural hyperkyphos<\/strong><br \/>\nKr\u00f6kningar mindre \u00e4n 60 grader i ton\u00e5ren<br \/>\nKr\u00f6kningar med 60-80 grader i ofullst\u00e4ndig ton\u00e5ren<\/p>\n<p>Kontrollen inneb\u00e4r r\u00f6ntgenbilder tagna p\u00e5 l\u00e5ng skolioskassett var sj\u00e4tte m\u00e5nad i st\u00e5ende position. Ett tr\u00e4ningsprogram kommer att rekommenderas om barnet lider av sm\u00e4rta.<\/p>\n<p><strong>Korsettbehandling:<\/strong>\u00a0Om missbildningen \u00e4r m\u00e5ttligt sv\u00e5r (60-80) och om patienten inte har avslutat skelettutvecklingen kan korsettbehandlingen rekommenderas med ett tr\u00e4ningsprogram. Heldagsanv\u00e4ndning av (20 timmar om dagen) korsetten rekommenderas tills maximal korrigering uppn\u00e5s. Anv\u00e4ndningen av korsetten kan reduceras till 12-14 timmar om dagen under det sista \u00e5ret under processen med att slutf\u00f6ra skelettutvecklingen. Korsetten b\u00f6r anv\u00e4ndas i minst 18 m\u00e5nader f\u00f6r att uppn\u00e5 en total och permanent korrigering.<\/p>\n<p><strong>Spinal fusion:<\/strong>\u00a0Den kirurgiska behandlingen kan rekommenderas om kyfotisk deformitet \u00e4r allvarlig (st\u00f6rre \u00e4n 80 grader) och om patientens ryggsm\u00e4rta \u00f6kar. Kirurgisk behandling s\u00e4kerst\u00e4ller en betydande korrigering och ingen postoperativ anv\u00e4ndning av korsett beh\u00f6vs. Ryggraden korrigeras och f\u00f6rl\u00e4ngs genom att skruvarna som \u00e4r placerade i kotorna f\u00e4sts med stavar. Operationerna g\u00f6rs ofta bakifr\u00e5n. Patienterna kan \u00e5terg\u00e5 till sina normala dagliga aktiviteter inom 4-6 m\u00e5nader.
