Kotorna ligger p\u00e5 en vertikal rak linje hos en normal och frisk person. Vid skolios \u00e4r ryggraden b\u00f6jd till h\u00f6ger eller v\u00e4nster sida och roteras ocks\u00e5 runt sin axel. S\u00e5dana kurvor kan vara en plats i ryggraden men kan ocks\u00e5 vara p\u00e5 flera platser i olika riktningar.<\/p>\n<h3>Symtom<\/h3>\n<p>Skolios \u00e4r vanligare i ton\u00e5ren. Det kan resultera i kosmetiska och o\u00e5terkalleliga skador p\u00e5 hj\u00e4rta och andningsorgan om det inte ingrips i tidiga skeden. Det \u00e4r viktigt f\u00f6r att identifiera sjukdomen att patienterna observerar sig sj\u00e4lva och formen p\u00e5 sin kropp f\u00f6r att m\u00e4rka tecknen.<\/p>\n<p>Dessa tecken inkluderar;<\/p>\n<ul>\n<li>Skillnad i niv\u00e5 p\u00e5 axlar<\/li>\n<li>Fram\u00e5t utskjutande en del av kotpelaren<\/li>\n<li>Utbuktande ben p\u00e5 ena sidan av ryggen (utbuktande utseende av skulderblad)<\/li>\n<li>H\u00f6ger eller v\u00e4nster avvikelse i kroppsbalans<\/li>\n<\/ul>\n<p>Hur diagnostiseras det?<\/p>\n<p>Diagnosen st\u00e4lls genom att en r\u00f6ntgenfilm som tas med st\u00e5ende st\u00e4llning t\u00e4cker hela kotpelaren och genom att ett eller flera tecken ovan vid klinisk unders\u00f6kning identifieras. En MR-unders\u00f6kning kr\u00e4vs f\u00f6r att identifiera orsaken efter att ha diagnostiserat skolios. Vinkeln m\u00e4ts mellan kotorna d\u00e4r kurvan b\u00f6rjar och slutar indikerad p\u00e5 frontal, posterior och lateral kotpelarradiografi tagen med st\u00e5ende position. Denna vinkel kallas Cobb.<\/p>\n<h3>Typer av skolios<\/h3>\n<p><strong>Medf\u00f6dd skolios<\/strong><\/p>\n<p>Medf\u00f6dd skolios uppst\u00e5r ofta i tidiga \u00e5ldrar och utvecklas i samband med en defekt i bildandet av ryggraden eller f\u00f6rening av revben. Det m\u00e5ste identifieras och behandlas i tidiga skeden. Det kan visa snabb utveckling beroende p\u00e5 typ av defekt.<\/p>\n<p><strong>Neuromuskul\u00e4r skolios<\/strong><\/p>\n<p>Detta kan orsakas av muskelpares i samband med neurologiska sjukdomar som stroke, cerebral pares eller muskeldystrofi. \u00c5terigen, tidig diagnos och behandling skulle l\u00f6sa progressiva defekter.<\/p>\n<p><strong>Idiopatisk skolios<\/strong><\/p>\n<p>Denna typ av skolios \u00e4r den vanligaste och den r\u00e5dande uppfattningen som nyligen enats om i de vetenskapliga publikationerna \u00e4r att sjukdomen \u00e4r \u00e4rftlig. Det upptr\u00e4der ofta hos unga flickor i en snabb utvecklingsperiod av ton\u00e5ren. Ungdomsskolios orsakar inte sm\u00e4rta och \u00e4r sv\u00e5r att identifiera. S\u00e5ledes kan skoliosen ha b\u00f6rjat flera \u00e5r innan den identifierades. Barnets ryggrad b\u00f6r kontrolleras regelbundet tills tillv\u00e4xten \u00e4r klar eftersom skoliosen kan uppst\u00e5 n\u00e4r som helst under ton\u00e5ren.<\/p>\n<h3><strong>Behandlingsmetoder<\/strong><\/h3>\n<p>Det best\u00e4ms utifr\u00e5n kurvans storlek och i vilket skede benutvecklingen \u00e4r. Ju tidigare behandlingen p\u00e5b\u00f6rjas desto h\u00f6gre \u00e4r framg\u00e5ngsfrekvensen.<\/p>\n<p><strong>Observation<\/strong><\/p>\n<p>Endast observation och periodiska kontroller b\u00f6r vara tillr\u00e4ckliga hos patienter med en grad mindre \u00e4n 20 och som n\u00e4stan har avslutat skelettutvecklingen.<\/p>\n<p><strong>Korsettapplikation<\/strong><\/p>\n<p>Korsetten \u00e4r avsedd att f\u00f6rhindra \u00f6kning av kurvan. Korsett \u00e4r s\u00e4rskilt effektivt d\u00e4r kurvan \u00e4r runt 30 grader och hos barn som fortfarande \u00e4r i utvecklingsstadiet. Korsetteffekten minskar d\u00e4r kurvan \u00e4r st\u00f6rre \u00e4n 40 grader och hos barn som har \u00e5r p\u00e5 sig att fullf\u00f6lja skelettutvecklingen. Kurvan kan \u00f6kas trots korsettbehandling. Graden av denna risk kan best\u00e4mmas till 99 % genom genetisk analys fr\u00e5n salivprov, och detta test utf\u00f6rs p\u00e5 v\u00e5rt center.<\/p>\n<p><strong>Kirurgi<\/strong><\/p>\n<p>Den kirurgiska behandlingen \u00e4r det enda alternativet d\u00e4r kurvan \u00e4r st\u00f6rre \u00e4n 40 grader och hos patienter som fortfarande \u00e4r i utvecklingsstadiet. Den kirurgiska behandlingen utf\u00f6rs framg\u00e5ngsrikt med implantat (skruvst\u00e4nger) placerade p\u00e5 ryggen och nedre delen av ryggen. \u00d6vervakning (neuromonitorering) av ryggm\u00e4rgsfunktioner under operationen \u00e4r en metod som \u00f6kar drifts\u00e4kerheten f\u00f6r patient och l\u00e4kare och anv\u00e4nds rutinm\u00e4ssigt p\u00e5 v\u00e5rt center.<\/p>\n<p>V\u00e5r klinik \u00e4r ett av centren som \u00e4r indicerat som ett referenscentrum \u00f6ver hela v\u00e4rlden f\u00f6r skoliosoperationer. 65 patienter med diagnosen idiopatisk skolios i ton\u00e5ren genomgick enbart skoliosoperation p\u00e5 v\u00e5rt sjukhus under 2012. Antalet ans\u00f6kta operationer f\u00f6r andra typer av skolios \u00e4r ganska lika.<\/p>\n<p>Operationsprocessen \u00e4r organiserad p\u00e5 ett extremt professionellt s\u00e4tt f\u00f6r att underl\u00e4tta f\u00f6r patienterna. Den operationsmetod som ska till\u00e4mpas best\u00e4ms av din l\u00e4kare baserat p\u00e5 typen av skolios och du f\u00e5r information om operationen i detalj. Endast en operation r\u00e4cker f\u00f6r att 90 % av patienterna ska f\u00e5 ett gynnsamt resultat. Patienterna har st\u00e5tt upp och g\u00e5tt en dag efter operationen. Varaktigheten av sjukhusvistelsen \u00e4r cirka 5 dagar.
